Bend the blades of the radiator so the screws will have something to actually screw in.

Just use hot glue or building silicone to "glue fix" the fan to the heatsink.

I continue to try to improve my debug kit with a 500gb SATA HDD. So when I try to format it I have this message:

    I use a toshiba HDD.

On a retail Xbox, error 16 indicates a loss of clock data and the dashboard file to set it is not found on the HDD.

Clock loss is a result of a discharged clock cap after removing AC power from the console. I see you've removed to clock cap so the instant you unplug the console, the clock date/time will be lost.

Power surge is something related to the usb-serial adapter or you are trying to power the homebrew card from the usb. Try to swap the usb adapter.

Yuo should power the module from xbox, not from the usb adapter. Disconnect the red wire.

Yes, they are polarized - the negative is marked by a band with "-" signs in it.

The polarity should be marked on the board - MS normally mark them both with a "+" on the positive pin and a filled area of the silkscreen near the negative terminal.

When you installed the old HDD, did the dashboard startup asking to set the date/time? If so, set it and don't unplug AC power when attempting to format the new larger HDD.

Can't tell as the images aren't the clearest to make out. The bottom cap solder point in the first image doesn't look too good to me and there seems to be a lot of flux left and possible solder beads splattered about on the PCB.

Last picture where do you think there's a leak? Too many caps to know which you think is having a problem.
