No hint in the project folder names or anything?
Nope. According XBEExplorer the debug file path for the file was C:\project\Xbox\mayhem\mayhem\Release\mayhem.exe

lol yeah. I laugh at how everything I find about this is just called Mayhem. I wonder who decided on the name.
Probably just one of those things that sounded good as a placeholder, not expecting anyone else to find it.
I had hoped that I (or someone else) would be able to figure out if this is a game or not. Guess we'll never know. At least we know the build date.
Any idea of the origin of the Devkit ? Can your friend tells you where he got it ?
Also, any sticker on the unit itself ?
Well it came on a DVT-4 that I got from @N64 freak a while back. But I don't know if it was originally on that Xbox. Only he knows that. But if it did then the console itself is dated January 7th 2003.
Oh and no there are no stickers on the unit besides the normal ones.
Don't know much about the console either. I bought it from someone in the US a few years back and the seller just mentioned the demos on there as a little bonus.
He didn't seem to know much about the origin off the console either.
I sadly can't help to find out where the console came from but all the demos that are on that console now have been on it as i got the console!
For some early builds of games I have, I have been able to get in touch with several developper just by looking at the "Windows" property of some files (right click, then properties). In the "Details" section, you sometimes have the name of the creator of the file, or the name of the company, especially with files that can be opened with Notepad. Have you been able to check this ?

Okay so I looked through the files and checked the properties and there where no names or companies listed.
I did however find out that in the game you can turn shadows on and off by pressing START and BACK. I also found out that in split screen mode if one player shoots sticky bombs at the other player and that other player drives towards the bombs, they die. It also causes the game to freeze and crash.
I am rendering the gameplay of me and my friend in split screen mode and I will upload it tonight. Its long though (like 21 min lol).
Sad that you didn't find more informations, but glad that you have been able to record your two player session.

EDIT: Whats ironic is if he opens the Mayhem split screen game with a second controller plugged in it plays fine.
Figure out where the guy was from, could give you some idea of the studios in the general area.
I messaged him via youtube. Hopefully I get a reply soon and I'm sure it will definitely narrow down the search.
Glad I could be of some use to you. I seen the photo of the game and thought that it look really familiar and remembered that YouTuber had that on a devkit at one point. Pretty cool that it managed to work its way here though xD
Maybe "Metal Shell"? Was going to be for Xbox, PS2, and PSP, but was cancelled. Looks kind of similar.
